DPC, HSA & FSA- What Employers Need to Know

Can employees use an HSA with Direct Primary Care?


Yes. Qualifying Direct Primary Care arrangements are now HSA-compatible.


Beginning January 1, 2026, individuals who otherwise qualify for an HSA may participate in certain qualifying Direct Primary Care arrangements without the DPC arrangement itself making them ineligible to contribute to an HSA.


HSA funds may also be used tax-free to pay qualifying periodic DPC membership fees.


For 2026, qualifying DPC fees are subject to applicable federal monthly limits. Employers and employees should consult their benefits, tax, or plan advisors regarding their specific coverage and eligibility.


What about FSA funds?


FSA reimbursement may depend on the employer’s specific plan documents and plan administrator. Employees should verify whether DPC-related expenses qualify for reimbursement under their particular Flexible Spending Account.

Does DPC replace our employees’ health insurance?

Can an employer pay for DPC membership?
